In accordance with the paragraphs 5.6. and 5.7. of the Article 5 of the Law of the Republic of Azerbaijan "On Personal Data" dated 11.05.2010 No. 998-IIIQ, I have no objection to "Azerbaijan Airlines” Closed Joint Stock Company for automated, as well as without the use of automated means, processing of my personal data provided by me for participation in the selection of candidates for a vacant position and declared during the interview, namely: for collection, recording, systematization, accumulation, storage (including copies of submitted documents containing my personal data), clarification (updating, alteration), extraction, use, transfer (distribution, submission, access), including transfrontier transfer, depersonalization, blocking, extraction, deletion of personal data, namely: surname, given name, patronymic, date and place of birth, sex, citizenship, phone number and e-mail address, place of residence and seat, qualification and place of employment, type of activity, marital status, photo and other data.

I take the responsibilty for providing false personal data and agree to confirm them during an in-person interview. I agree that in case of inconsistency of the stated information, the management of “Azerbaijan Airlines” CJSC may not consider my candidacy and hire me for a job.
